Create your account, set up sign-in security, and write your first note. Who this is for: New users · Plans: All plans · Where: Web (app.chartnote.com), mobile app

1. Create your account

1

Go to app.chartnote.com/welcome and sign up with your email, or with Google.

2

Enter the verification code we email you.

3

Complete Complete Your Profile.

4

On Let's Keep Your Account Safe, choose SMS, Authenticator App, or Not now. Google sign-up skips this step. If you were invited to a team that requires MFA, Not now isn't available. Email codes aren't offered here, but you can set them up later in Settings → Account & Security.

5

Download the mobile app if you want to record on your phone, then go to Home.

2. Create a note

Home asks “What would you like to work on?” Choose how to document:
  • AI Scribe: click Start a visit. AI Scribe records the visit and writes a structured note. See Record patient visits with AI Scribe.
  • Voice Chart: click Dictate a note to dictate a summary after the visit. The note is written in the language you dictate.
  • Plain Note: start from a template or blank, and type or dictate.
  • Scratch Pad (in the left sidebar): paste or dictate clinical information, then convert it into a note using your selected template.
Every new note is saved as a draft right away, so you can continue on any device.

3. Finish later, from any device

Audio syncs after each pause. Click Finish later to get a short Highlight of the visit so far, then resume recording or click Generate Note when you’re ready. Highlights are free.

4. Review, copy and send to your EHR

When the card shows Ready, open the note to edit it, copy it, or download it as a PDF. Use the Chrome extension to load notes into your EHR.

Save time with templates and snippets

  • Templates shape how your notes are organized. Your own templates are listed first.
  • Snippets insert common text. Type @ followed by the snippet abbreviation.
  • Explore templates and snippets shared by the community.
